Bring medication and allergy details that your clinician can identify accurately.

Prepare the names, doses and reasons for your medicines, plus a description of previous allergic reactions. Include non-prescription products and supplements. Do not stop prescribed medicine on your own to prepare for a cosmetic appointment.

Record the reaction rather than only the label

For an allergy or previous adverse reaction, explain the substance, what happened and when it occurred. If you do not know a medicine’s English name, bring its packaging or a clear product record for the clinician to review. Include previous reactions to injections or numbing products. Ask the clinic how it wants to receive sensitive information; the public website is not a medical-record upload service.
