Bring dates, product or device names, treatment areas and any problems from previous procedures. This helps the clinician assess a new proposal without guessing what was used or where it was performed.
Build a short treatment timeline
List the most recent procedures first and include any implants or injectable treatments relevant to the area being discussed. If you have a treatment record, bring it; if a detail is unknown, label it unknown rather than guessing from a brand you remember. Describe any ongoing symptoms separately from the result you liked or disliked. Ask whether missing information needs to be obtained before a decision.
